Abstract:

At the Children Emergency in Children Hospital the burns are rinse with cool water for 15 minutes. The blisters are lanced and debridement of devitalized tissue are done only when blister has burst before transparent film is applied. Patient is then given an early appointment within the next 36 hours  to the Plastic outpatient clinic.

Report on return visits for dressing reapplication over previous film dressing due to leakage and too much exudates/blisters, film dressing loosen including degree of difficulty to remove the transparent films were noted.  Exposed burns areas with dried exudate over trunk, degree of maceration due to collection of exudates under film dressing especially of hands and feet were photodocumented before and after debridement.

Plastic team and Children Emergency collaborated to reduce reattendance,  improve patient satisfaction and reduce procedural distress as an integral part of burns procedural management.

Method

Trial use of foam dressing was initiated. Criterias called for minimal maceration and leakage, superior absorption and exudate retention, easy to apply, comfort and removal without causing pain to the child and more importantly, meet the need in our local humidity context.

Formal lecture followed by demonstration/training session on the use of foam dressing were provided to all the nurses in Children Emergency.

Results

Degree of maceration, exposed wounds from active toddlers were reduced. No further return visits to Children Emergency within 24 hrs.

Patient were comfortable, less pain wass observed when foam dressing is removed . Use of foam dressing following tissue cooling is adopted .
